Dear Clients,

 

Amistad Family Services is writing you to notify you of a recent data incident that may have resulted in unauthorized access to your personal information. You are receiving this letter because you are a client who currently or previously has contracted to receive care from Amistad Family Services.  Amistad Family Services deeply regrets that this happened, and we recommend you closely review the information in this letter.

 

What Happened?

On October 26, 2021, an incident occurred in which a lockbox was stolen from our facility.  A backup copy of patient data from 2015-2018 was in this secured box.  The file containing the data was non-descript, not in a human-readable format, and a very specific set of circumstances would be required to reconstruct the data into something usable.  However, out of an abundance of caution, we are notifying anyone whose personally identifiable information MAY have been affected by this breach.

 

What Information was Involved?

Files containing client information MAY have been impacted. Affected data elements may have included a client’s name, date of birth, street address, social security number (only in some instances), and client identification number. At this time, we have no evidence that any personal information has been misused.

 

What Are we Doing?

Back up information is no longer stored on site. Amistad Family Services has installed deadbolt locks on all doors, additional security cameras, and additional alarm systems.  We have hired Experian to assist in notifying each individual client.
